About the Role
The Clinical Transformation Specialist is Innovaccer's on-the-ground clinical presence at Atlantic Health Group. This role owns clinical adoption, eCQM execution, and quality measure improvement at the point of care — beginning where implementation ends and outcomes begin.
Atlantic Health Group serves 870 physicians across 50+ sites, with 200,000 attributed lives under 11 payer contracts. With a renewed four-year engagement inclusive of eCQM, InNote, and InGraph, Atlantic is a high-priority strategic account. This role ensures the platform drives the clinical outcomes the partnership was built on.
A Day in the Life
eCQM Execution & Quality Measure Improvement
- Lead eCQM workflow sessions with clinical liaisons, care managers, and providers across Atlantic practice sites
- Translate HEDIS, CMS, and payer-specific quality measure requirements into actionable, site-specific clinical workflows
- Monitor quality gap closure rates across InNote and proactively identify sites at risk of missing submission targets
- Partner with Atlantic's clinical liaison team to coordinate gap closure campaigns across all in-scope measures
- Validate that eCQM measure logic aligns with how providers document at the point of care — and close the gap when it doesn't
InNote Clinical Adoption
- Conduct structured InNote workflow assessments across Atlantic sites to identify adoption gaps by site, provider, and measure type
- Design and deliver site-specific training and workflow integration sessions for clinical staff and providers
- Identify and resolve workflow friction points that reduce provider willingness to act on InNote prompts
- Track InNote utilization, provider engagement, and gap closure per site on an ongoing basis
- Represent InNote to frontline clinical staff as clinical decision support, not administrative overhead
Clinical Workflow Design & Change Management
- Map current-state clinical workflows at each Atlantic site and identify where Innovaccer tools are creating friction or being bypassed
- Redesign workflows collaboratively with practice leadership to embed InNote and InGraph into existing clinical routines
- Develop site-specific SOPs, clinical tip sheets, and workflow guides tailored to how each practice operates
- Lead change management initiatives alongside Atlantic's VP-level and program leadership, building internal champions
- Facilitate clinical performance reviews with Atlantic leadership to demonstrate progress and align on next priorities
Performance Monitoring & Reporting
- Track and report weekly on eCQM submission readiness, InNote utilization by provider and site, and quality gap closure rates
- Identify underperforming sites early and escalate with recommended interventions before milestones are at risk
- Contribute to quarterly clinical outcome reports for Atlantic executive leadership
- Serve as the primary clinical voice in account status conversations between Innovaccer CS and Atlantic leadership
Collaboration & Cross-Functional Support
- Coordinate with the Data Acquisition & Technical Readiness track to ensure data quality issues do not block clinical adoption
- Partner with Innovaccer's CSD and CEM teams to maintain alignment on account status, blockers, and escalations
- Surface platform gaps and workflow friction points back to Product through structured feedback channels
- Document clinical program configurations, protocol decisions, and workflow designs for continuity and scale
What You Need
- Active clinical licensure (RN, NP, PA, LCSW, or equivalent clinical credential)
- 3+ years of experience in clinical practice, care management, population health, or ambulatory clinical operations
- Direct experience with eCQM, HEDIS, or CMS quality measure programs — including how measures are documented and submitted
- Experience working with Epic or comparable EHR systems in a multi-site ambulatory environment
- Demonstrated ability to drive clinical workflow change with frontline providers and clinical staff
- Strong communication and facilitation skills — comfortable presenting to frontline staff and executive leadership in the same week
- Ability to operate independently in a client-embedded, on-site model with minimal day-to-day supervision
Preferred Qualifications
- Background in healthcare technology, clinical informatics, or population health platforms (Innovaccer, Epic, Arcadia, or similar)
- Experience in an ACO, value-based care, or MSSP environment
- Prior experience in an embedded or client-facing clinical consulting role
- Knowledge of quality incentive payment structures and how measure performance translates to financial outcomes for a physician network
- New Jersey or Northeast-region network familiarity a plus

- eCQM measures documented and submission-ready across all Atlantic practices by June 2026 deadline
- InNote prompt acceptance maintained at 98%; clinical response depth improving month-over-month
- Quality gap closure rate sustained at 6.7% improvement with targeted gains per site
- 100% of active Atlantic sites engaged with a structured CT workflow session within 90 days
- Provider engagement increased by 25% in bottom-quartile sites within 90 days
