POSITION PURPOSE:
Under the supervision of the Clinic Manager, the School Based Health Center Registered Nurse (the “SBHC-RN) has primary responsibility, authority and accountability for safely and effectively delivering appropriate medical services to persons qualified to receive services at Aviva Health’s School Based Health Clinic (SBHC), educating SBHC patients and their families about medical conditions and healthy lifestyle choices, and establishing and following clinical policies, procedures and practices that are appropriate for the needs of SBHC patients and that comply with State and Federal criterion for School Based Health Centers. This position will also be interfacing with Oregon State agencies that regulate School Based Health Centers.
ESSENTIAL FUNCTIONS OF THE JOB:
- Meet all mandatory continuing education and proof of professional growth requirements of appropriate regulatory agencies, certifying boards and Aviva Health.
- Competently evaluate and treat non-acute and chronic health conditions in school aged and adult patients.
- Competently and ethically deliver comprehensive nursing services appropriate to the SBHC clinical setting.
- Employ the best practices of the nursing profession to ensure that SBHC patients receive the highest quality of care.
- Communicate diagnoses and treatment instructions to SBHC patients and their families in a clear, concise, and meaningful way.
- Administer injections, screens and tests as ordered or prescribed by differently qualified providers.
- Identify the need to refer SBHC patients for tests and treatments not otherwise appropriate to be performed in the SBHC clinic setting.
- Recognize the need to refer patients to differently qualified providers.
- Maintain accurate and complete medical records in a timely fashion for all SBHC patients.
- Work collaboratively and cooperatively with colleagues and co-workers in the clinical setting.
- Accept the supervision of and adhere to the directives of the SBHC Provider, the Chief Medical Officer (CMO) of Aviva Health, Chief Executive Officer (CEO) or the CEO’s designee.
- Adhere to and model the highest standards of the nursing profession at all times.
- Meet Aviva Health’s standards for excellence in the provision of healthcare services for its patients.
- Develop a program of health education for SBHC patients and their families that includes the basic principles of a healthy lifestyle and health risk reduction.
- Deliver the health education program to SBHC patients and their families in a clear, concise, and meaningful way.
- Utilize approaches to and techniques of instruction that support and enhance the health and well-being of SBHC patients and their families.
- Effectively communicate through written material and oral presentations to groups or individuals.
- Adheres to all laws and regulations governing SBHC clinics.
- Adheres to the terms and conditions of any agreements between Aviva Health and the School District concerning the services provided in the SBHC clinic.
- Adheres to all Aviva Health clinical policies, procedures and practices that are appropriate for the needs of SBHC patients and their families.
- Treats SBHC patients and their families with integrity, empathy, and compassion.
- Triages phone calls and walk-in patients for acuteness of need and/or priority of intervention and documents assessment information of all activities.
- Rooms patients that are on differently qualified providers’ schedules at the SBHC.
- Performs nurse visits for students at the SBHC per standing orders.
- Follows-up with SBHC patients seen at the SBHC clinic.
- Protects confidential client information and only shares information in a manner that is consistent with Aviva Health Policy and current law (both HIPAA and FERPA).
- Oversees the operation, accountability for, and maintenance of clinic equipment.
- Attends staff meetings, in-service meetings and participates in Aviva Health committees and task force activities as required.
- Attends all required webinars concerning the SBHC Program. Works collaboratively and cooperatively with colleagues and co-workers.
- Treats the SBHC staff with integrity, empathy, and compassion.
- Acts with integrity, empathy, and compassion in and outside of the SBHC clinical setting.
- Promotes a strong belief in Aviva Health's philosophy, purpose, and ideals.
- Performs other duties as assigned by supervisor.
QUALIFICATIONS
- Possess an active and unencumbered Oregon Registered Nurse license (BSN or MSN preferred but not required).
- Three to five years of ambulatory care experience, preferably in an FQHC or Public Health setting.
- Experience with electronic health record technology (Athena Health EMR preferred).
- Possess proven leadership experience, excellent patient service skills, team building and collaboration skills.
- Ability to work independently, multitask, prioritize, delegate, be detail oriented and organized, work effectively under pressure, and meet deadlines.
- Possess excellent computer and organizational skills.
- Demonstrate knowledge of health care office procedures and patient flow.
- Possess strong administrative skills.
- Have excellent communication and interpersonal skills, including oral and written comprehension and expression of the English language.
- Possess the ability to comprehend and analyze complex concepts and data; strong problem-solving skills and sound judgment.
- Ability to lead by example and delegation, having a strong work ethic.
- Demonstrate respect for and sensitivity to cultural/social differences in interactions with SBHC patients and their families as well as co-workers and colleagues.
- Successful completion of any and all pre-employment screens and credentialing requirements in accordance with Aviva Health policies and procedures.
