Key Responsibilities:
- Provide direct patient care in accordance with established nursing standards and facility policies
- Administer medications and treatments as prescribed by physicians
- Monitor and document patient conditions, reporting any changes to the RN or physician
- Assist residents with daily living activities, ensuring comfort and safety
- Maintain accurate and timely patient records
- Collaborate with interdisciplinary teams to deliver comprehensive care
- Ensure compliance with all regulatory and safety standards
Requirements:
- Active and valid Wisconsin LPN license (must be maintained throughout assignment)
- Current CPR/BLS certification
- Minimum 1 year of related Long-Term Care experience preferred
- Strong attention to detail and accuracy
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
