How to Get Visa Sponsorship in Internist
Target academic medical centers and teaching hospitals
University-affiliated hospitals sponsor significantly more physicians than private practices. They have dedicated immigration departments and established processes for J-1 waiver programs, H-1B petitions, and EB-2 applications for international medical graduates.
Complete USMLE Steps 1, 2, and 3 before applying
USCIS requires evidence of medical competency for physician visa petitions. Passed USMLE exams strengthen your specialty occupation argument and demonstrate qualification for internist roles requiring advanced medical knowledge and training.
Emphasize board certification or eligibility
American Board of Internal Medicine certification or clear pathway to certification strengthens visa applications. Employers prefer candidates who can demonstrate specialty training completion and board eligibility within standard timelines.
Research employer's J-1 waiver program participation
Many internist positions start as J-1 exchanges with waiver requirements. Verify if employers participate in Conrad 30, VA, or other waiver programs before accepting offers requiring specific service commitments.
Document subspecialty training for competitive advantage
Fellowship training in cardiology, gastroenterology, or other internal medicine subspecialties can differentiate your application. Specialized skills may qualify for EB-1B extraordinary ability or EB-2 national interest waiver pathways.
Prepare for PERM labor certification timeline
Hospital PERM processes for physicians typically take 18-24 months. Plan your visa strategy around these timelines, especially if transitioning from J-1 or nearing H-1B maximum stay limits.

Get Access To All JobsFrequently Asked Questions
Do international medical graduates need specific degrees for internist H-1B sponsorship?
Yes, you need a medical degree from an accredited institution plus completed residency training in internal medicine. ECFMG certification is required to practice in the U.S.. Three-year medical degrees may require additional credential evaluation to meet H-1B specialty occupation requirements for physician roles.
What visa options work best for internist positions?
H-1B is most common for hospital-employed internists, while J-1 exchange visitor status works for residency and some fellowship positions. EB-2 provides permanent residency path through PERM labor certification. J-1 physicians often need waiver programs before H-1B eligibility.
How do J-1 waiver requirements affect internist sponsorship?
Many international medical graduates enter on J-1 visas with two-year home country requirement. You'll need a waiver through Conrad 30, VA, or other programs before H-1B eligibility. Some employers specifically recruit for J-1 waiver positions in underserved areas.
Are internist positions considered specialty occupations for visa purposes?
Yes, internist roles clearly qualify as H-1B specialty occupations requiring advanced medical knowledge, completed residency training, and board certification or eligibility. USCIS recognizes internal medicine as requiring specialized education and training that general bachelor's degrees cannot provide.
What approval rates do physicians see for H-1B petitions?
Physician H-1B petitions have historically high approval rates, often above 90%, due to clear specialty occupation requirements and well-documented qualifications. However, USCIS may request additional evidence for newer residency programs or less established healthcare employers without strong sponsorship track records.
What is the prevailing wage requirement for sponsored Internist jobs?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
