Mental Health Kokua (MHK) is hiring a full-time Care Coordinator to support residents living in the Kauhale supportive housing community at the Iwilei Center on Oahu. This role focuses on helping residents maintain stability by coordinating health services, appointments, and essential resources that support recovery and independent living.
In this role, you will work closely with residents, case managers, and community providers to ensure individuals have consistent access to care, transportation, benefits, and wellness supports. This position is ideal for individuals with experience in behavioral health, housing programs, or social services who want to make a meaningful difference in a supportive housing environment.
What You’ll Do (Responsibilities)
- Care Coordination: Schedule and track medical, psychiatric, and behavioral health appointments and support follow-through
- Service Navigation: Assist residents with benefits enrollment, transportation, and access to community services
- Resident Engagement: Conduct regular check-ins and encourage participation in wellness activities and recovery supports
- Safety & Crisis Response: Monitor resident well-being and respond appropriately to emergencies or concerns
- Documentation: Maintain accurate records and update client information in HMIS, EMR, and internal systems
- Team Collaboration: Coordinate with program staff, case managers, peer specialists, and external providers
- Program Support: Assist with on-site service events, wellness activities, and recovery-focused programming
Care Coordinator Qualifications
- Minimum: High school diploma or equivalent, plus 2 years in behavioral health, housing, or social services; OR Associate’s degree with 1 year experience
- Preferred: Bachelor’s in Social Work, Psychology, Human Services, or related field; experience with HMIS or care coordination platforms
- Current TB clearance, CPR, and First Aid certification (or ability to obtain)
- Must pass a criminal background check and drug screening
- Able to lift up to 25 lbs and respond to facility-related needs
