Care Manager Jobs in Brooklyn: Frequently Asked Questions
How do I get a care manager job in Brooklyn?
Focus on Brooklyn's dense network of community health centers, federally qualified health centers, and Medicaid managed care plans, which consistently post the most openings. Neighborhoods like Brownsville, East New York, and Sunset Park have high concentrations of these employers. Candidates with case management experience in behavioral health or chronic disease management, familiarity with New York State Medicaid populations, and bilingual skills in Spanish, Haitian Creole, or Chinese hold a clear edge in this market.
Which companies hire care managers in Brooklyn?
Employers hiring care managers in Brooklyn right now include Advance Care Alliance New York, Hamaspik Choice, and mercyFirst, based on current listings on Migrate Mate as of August 2026. Brooklyn's hiring landscape is shaped by a mix of large hospital systems, nonprofit social service agencies, and managed long-term care plans serving the borough's diverse communities.
Are there remote care manager jobs in Brooklyn?
Yes, but with limits, since care management often involves direct client contact, home visits, or in-person coordination with clinical teams. About 69% of care manager openings tied to Brooklyn are remote or hybrid as of August 2026, reflecting an employer preference for at least some on-site presence. The most remote-eligible functions tend to be utilization review, telephonic care coordination, and documentation-heavy case management roles with managed care organizations.
How can I get a care manager job in Brooklyn with little or no experience?
The most realistic entry path is through a care coordinator or community health worker role at one of Brooklyn's many federally qualified health centers or nonprofit social service agencies, which routinely hire candidates with a bachelor's degree in social work, health education, or a related field. Agencies serving Medicaid populations in Brownsville, East Flatbush, and Bushwick often prioritize candidates with lived community experience over formal case management credentials. Volunteering with community health programs or completing a field placement in a Brooklyn clinical setting also strengthens a local candidacy meaningfully.
