Case Manager – Parent Support & Family Services
JC Wellness · Alameda & Contra Costa Counties, CA (hybrid, community-based) · $22–$25/hour, based on experience · Full-time or part-time
JC Wellness supports Regional Center families and adults with intellectual and developmental disabilities. We're hiring Case Managers to provide hands-on Parent Support and case management, helping parents build skills, connect to resources, and reach their goals.
What you'll do
- Provide direct Parent Support Services in the home and community, focused on parenting and child-centered goals
- Manage a caseload: coordinate services, track progress, and connect families to community resources
- Work with Regional Center of the East Bay service coordinators on each family's plan
- Document every visit and write progress reports
- Complete incident reports and attend team meetings
What you'll need
- Associate's degree (AA) or college credits in social work, human services, psychology, child development, or a related field
- Experience with families or people with I/DD is a plus, not required
- CPR/First Aid; Live Scan, DOJ and FBI clearance
- Valid CA driver's license, clean DMV record, reliable vehicle and auto insurance
- Fluent English; bilingual candidates preferred
