Position Summary

Responsibilities
- Perform comprehensive nursing assessments and evaluate patients' medical conditions, functional status, and care needs.
- Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team.
- Provide skilled nursing services, including medication management, wound care, disease management, and patient monitoring, as indicated by the plan of care.
- Monitor patient progress and identify changes in condition, communicating concerns and coordinating appropriate interventions.
- Educate patients and caregivers on diagnoses, medications, treatment plans, disease processes, and safety precautions.
- Complete accurate, timely, and compliant clinical documentation in accordance with agency policies and applicable regulations.
- Coordinate care with physicians, therapists, social workers, and other healthcare professionals to promote continuity of care.
- Participate in patient admissions, routine visits, recertifications, resumption-of-care visits, and discharge planning as assigned.
- Support patient-centered care focused on safety, independence, and improved health outcomes.
- Travel to patient homes throughout the Northern Orange County service area.
Qualifications
- Current, active California Registered Nurse (RN) license in good standing.
- Previous home health nursing experience is required.
- Experience with home health assessments, individualized care planning, and clinical documentation.
