- Begin discharge planning early in placement to ensure smooth transitions to home, independent living programs, etc.
- Build positive relationships with youth, families, schools, therapists, and community partners.
- Assist with educational planning, life skills, and transition services.
- Maintain accurate documentation and ensure compliance with agency and DFPS requirements, including keeping youth files up-to-date.
- Transport youth to medical, dental, mental health, educational, court, and other scheduled appointments using agency vehicles.
- Help schedule appointments and communicate with providers, staying abreast of follow-up appointments as well.
- Participate in treatment team meetings and collaborate with direct care staff.
- Support youth using trauma-informed practices and assist during crisis situations when needed.
- Maintain ethical boundaries, cultural competence, and a trauma-informed approach in all interactions.
- Bachelor's degree in Social Work, Psychology, Education, Human Services, or a related field (or qualifying equivalent experience).
- A minimum of 1 to 3 years of related work experience required, but 3-5 years preferred.
- Minimum age of 23 years.
- Valid Texas driver's license with a safe driving record.
- Ability to pass required background checks.
- Experience working with youth, foster care, residential treatment, or behavioral health preferred.
- Strong communication and organizational skills.
- Compassionate, patient, and dependable.
- Ability to work independently while collaborating with a multidisciplinary team.
