YOUR ROLE AND RESPONSIBILITIES
Responsibilities Include:
- Providing clinical leadership and support to nurses and therapists in the field
- Reviewing OASIS documentation and ensuring Medicare compliance
- Managing care plans (485s) and clinical documentation workflows
- Supporting onboarding, training, coaching, and performance development
- Serving as a clinical resource for triage and patient care guidance
- Driving quality outcomes and best practices across the office
BASIC QUALIFICATIONS
Required Qualifications:
- Active Registered Nurse (RN) license
- Minimum 2 years of nursing experience
- Recent Home Health experience required
- Strong hands-on OASIS experience
- Knowledge of Medicare regulations and clinical documentation
- Excellent leadership, communication, and organizational skills
