Position Highlights:
- Full Time Opportunity
- Join team of 1 Physician
- Office Hours: Monday – Friday 8:00am – 5:00pm
- Embedded care team support (RN Care Managers, MSWs, CHWs, Diabetes Educator, PharmD)
- Lab, pharmacy & interpreter services
- Located less than 2 blocks from our main hospital, Atrium Health Carolinas Medical Center
- Access to additional System level care management resources and virtual technology
Ideal Candidate:
- Primary Care experience is preferred
