Local Hospice Company is looking for a compassionate and dedicated Hospice RN Case Manager to join our field-based team in Saint Charles, IL. In this rewarding visiting nurse role, you'll be at the heart of delivering exceptional end-of-life care directly to patients and families in the comfort of their own homes. Responsibilities:
- Manage a caseload of hospice patients, conducting in-home assessments and developing individualized care plans
- Perform skilled nursing visits to monitor patient symptoms, pain management, and overall comfort
- Collaborate with an interdisciplinary team including physicians, social workers, chaplains, and aides to coordinate holistic patient care
- Educate and support patients and their families on disease progression, medication management, and comfort care techniques
- Document clinical findings, care plans, and visit notes accurately and in compliance with hospice regulations
- Ensure compliance with Medicare/Medicaid hospice guidelines and agency policies
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
- Participates in on-call duties as defined by the on-call policy.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Attends bi-monthly interdisciplinary group meetings.
Requirements:
- Active RN license in the state of Illinois (required)
- Minimum 1-2 years of clinical nursing experience; hospice, palliative care, or home health experience preferred
- Strong knowledge of pain and symptom management in end-of-life care
- Maintains a current CPR certification.
- Excellent communication and interpersonal skills with patients, families, and care teams
- Ability to work independently in a field-based, self-directed environment
- Valid driver's license and reliable transportation for home visits in the Saint Charles, IL area
- Proficiency with electronic medical records (EMR) documentation
- Empathetic, patient-centered demeanor with a commitment to dignity and compassionate care
Pay: $55.00 - $100.00 per hour
Expected hours: 14.0 – 22.0 per week
