H-1B Visa Licensed Therapist Jobs
Licensed Therapist roles qualify as H-1B specialty occupations when the position requires a master's degree or higher in counseling, social work, or a related clinical field. Many hospital systems, behavioral health networks, and community mental health centers sponsor H-1B petitions for licensed clinicians, particularly where state licensure requirements reinforce the degree-specific nature of the work.

INTRODUCTION
NaphCare is hiring a full-time Licensed Therapist (LCSW, CPC, LMFT) to join our Jail Based Competency Treatment (JBCT) Program, THRIVE, that operates as a 30-bed adult competency to stand trial restoration and evaluation unit at Washoe County Detention Facility in Reno, NV. The hourly rate for this position is $52.25/hr. This is a unique opportunity to work in a structured, therapeutic, and team-driven environment where you can directly impact patient outcomes and help individuals with serious mental illness restore competency and move forward with their lives and legal cases.
The Therapist will conduct psychosocial assessments, develop individualized treatment plans, evaluate treatment goals, and provide individual and group therapy using psychoeducation, evidence-based practices, and skill-building interventions. This role supports patients in improving psychiatric stability, functioning, and overall well-being.
THRIVE provides specialized behavioral health services for patients found incompetent to stand trial (IST) and have been remanded to inpatient restoration treatment. The unit is a therapeutic environment, providing intensive day treatment services. By providing patient-centered, evidenced-based, and holistic care, THRIVE seeks to eliminate the need for these patients to transfer to await further delays in their legal case by having to await transfer to a hospital. The ultimate aim is to assist defendants in moving forward with their legal case and with their lives in a healthier place.
The Therapists work with a team of psychiatric providers, psychiatric nurses, forensic examiners, forensic educator, group facilitator, and data coordinator led and supported by a program director.
Responsibilities
Position Description:
- Serve as a contributing member of the multidisciplinary treatment team
- Conduct and document initial (or updates to existing) psychosocial assessments
- Perform and document treatment plans and treatment team review notes
- Maintain an individual caseload of THRIVE patients for individual treatment and documentation
- Conduct brief daily check-ins with individual caseload
- Provide group therapeutic and psychoeducational services
- Conduct crisis intervention and suicide/safety risk assessments as warranted
- Assist in outcomes assessments of the THRIVE patients
- Coordinate with health and correctional staff regarding patients’ mental health care
- Participate in educational and training activities related to mental health care for patients, security personnel, and healthcare staff
- Demonstrate knowledge of DSM-V-TR criteria and mental health practice standards.
BASIC QUALIFICATIONS
Position Requirements/Preferred Experience:
- Masters level therapist with active, unrestricted licensure as a clinician (LCSW, LMHC, LMFT, LPC, etc.) in the state of Nevada
- Knowledge of the DSM-V-TR criteria and conditions

Verify your license transfers across states
H-1B approval is tied to the work location, and state licensure requirements differ significantly. Confirm your credentials meet the destination state's Licensed Professional Counselor or LMFT standards before targeting employers there, since an unlicensed clinician can't legally practice.
Target employers with behavioral health divisions
Hospital systems, managed care organizations, and federally qualified health centers file H-1B petitions far more consistently than private group practices. Filter your search by employer type, larger organizations have dedicated HR infrastructure and existing relationships with immigration counsel.
Use Migrate Mate to find verified H-1B sponsors
Search Migrate Mate to identify employers with confirmed LCA filing history for Licensed Therapist and related clinical roles. DOL Labor Condition Application data shows which organizations have sponsored positions matching your occupation code, so you're targeting employers with a real track record.
Document your supervised clinical hours early
Many states require 2,000 to 4,000 post-degree supervised hours for full licensure. Employers filing your H-1B petition need to confirm you meet the specialty occupation threshold, so having organized records of your supervised experience strengthens both the petition and any USCIS Request for Evidence response.
Understand how prevailing wage affects your offer
Your employer must certify your salary meets the DOL prevailing wage for your occupation and work location before filing the LCA. Run the OFLC Wage Search using the SOC code for your specific therapist role to know what wage Level I through IV looks like in your target city.
Flag telehealth roles that span multiple states
If a remote Licensed Therapist position requires you to hold licenses in multiple states, your employer may need to file separate LCA certifications for each work location. Clarify the practice jurisdiction structure during the offer stage, not after the petition is already filed.

Find Licensed Therapist JobsLicensed Therapist H-1B Visa: Frequently Asked Questions
Does a Licensed Therapist role qualify as an H-1B specialty occupation?
Yes, provided the position requires at minimum a master's degree in a directly related field such as clinical psychology, counseling, or social work. USCIS evaluates specialty occupation on a role-by-role basis. Positions that accept a general bachelor's degree as sufficient, without a field-specific requirement, are at higher risk of a Request for Evidence or denial.
How does state licensure affect H-1B sponsorship for therapists?
Your employer's H-1B petition is tied to the specific work location, and most states require independent clinical licensure before you can practice unsupervised. If your license is from a different state, the employer may need to delay your start date or structure your role under supervision while you complete reciprocity or endorsement requirements, which can complicate petition timing.
Can a private therapy group practice sponsor an H-1B?
Technically yes, but smaller practices rarely have the infrastructure to manage the filing process. The employer must have an IRS EIN, demonstrate financial capacity to pay prevailing wage, and work with immigration counsel to prepare the LCA and I-129 petition. Most successful sponsorships for therapists come from hospital systems, large behavioral health networks, or community mental health organizations.
What happens to my H-1B status if my clinical license lapses?
A lapsed license creates a direct problem because the H-1B approval is partly based on your ability to perform the specialty occupation as described in the petition. If you can no longer legally practice in the work location state, your employer may be required to notify USCIS, and you could lose H-1B status. Renew your license well before expiration and notify your employer's immigration team immediately if any issue arises.
How do I find Licensed Therapist jobs where employers already sponsor H-1B visas?
Migrate Mate filters employers based on verified DOL Labor Condition Application filing history, so you can identify organizations that have sponsored Licensed Therapist and related clinical roles. This is more reliable than asking during cold outreach, since LCA data is a matter of public record and reflects actual sponsorship activity rather than a recruiter's best guess about company policy.
