Job Summary
Provides psychosocial assessment, counseling, advocacy, and coordination of community resources for patients and families. Supports continuity of care, discharge planning, and emotional well-being in the hospital setting.
Minimum Qualifications
- Bachelor’s degree in Social Work required
- Licensed or license-eligible per state requirements
- Basic Life Support (BLS) certification
- Knowledge of community resources and discharge planning
Preferred Qualifications
- Hospital or healthcare social work experience preferred
Essential Duties & Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Provide counseling, crisis intervention, and emotional support
- Coordinate discharge planning and post-acute community resources
- Collaborate with physicians, nurses, and interdisciplinary care teams
- Advocate for patient rights and continuity of care
- Maintain accurate, timely clinical documentation
Physical Requirements
- Ability to stand and/or walk for extended periods.
- Frequent bending, reaching, squatting, and kneeling.
- Lift, carry, push, and pull up to 50 lbs independently; >50 lbs with assistance/devices.
- Assist with patient repositioning and transfers using proper body mechanics and equipment.
- Manual dexterity for clinical procedures and computer use; adequate vision, hearing, and speech.
