Zoya Care Health Home is seeking a compassionate, dependable, and motivated Care Coordinator who is passionate about helping individuals and families navigate complex healthcare and social service systems.
This is more than a desk job. As a Care Coordinator, you will work directly with high-risk Medicaid members in their homes and communities, helping them overcome barriers to healthcare, housing, behavioral health services, and other essential resources.

What You’ll Do
Member Outreach & Engagement
- Conduct in-person, telephone, and community-based outreach to assigned members.
- Build trusting, respectful relationships with members and their support systems.
- Educate members about Health Home services and available community resources.
- Engage members who may be difficult to reach or have complex needs.
- Provide culturally responsive and person-centered support.
Assessment & Care Planning
- Complete comprehensive assessments of member needs, including:
- Physical and behavioral health
- Mental health
- Substance use
- Housing and food security
- Transportation
- Social and environmental needs
- Other social determinants of health
- Develop individualized, person-centered care plans in partnership with each member.
- Monitor progress and update care plans as member needs change.
Care Coordination & Advocacy
- Coordinate services with:
- Primary care providers
- Behavioral health providers
- Substance use treatment programs
- Hospitals and specialty providers
- Housing agencies
- Community-based organizations
- Other healthcare and social service partners
- Help members schedule and attend important appointments.
- Provide appointment reminders and follow-up.
- Assist members in overcoming barriers to accessing healthcare and community resources.
- Advocate for members and help connect them with appropriate services.
Transitional Care
- Support members following hospital or emergency department visits.
- Assist with discharge planning and follow-up appointments.
- Help ensure continuity of care and communication between providers.
- Follow up with members to identify and address barriers after transitions in care.
Health Education & Self-Management
- Provide education and support related to:
- Medication adherence
- Chronic disease management
- Healthy lifestyle choices
- Preventive care
- Self-management skills
- Encourage members to take an active role in their health and wellness.
Documentation & Compliance
- Document member contacts, interventions, referrals, and outcomes accurately and within required timelines.
- Maintain complete and compliant electronic member records.
- Complete documentation within 24 hours of member contacts whenever required.
- Use electronic care management and documentation systems.
- Maintain confidentiality and follow all applicable program, privacy, and regulatory requirements.
Team Collaboration
- Participate in team meetings, case conferences, and care coordination activities.
- Communicate effectively with supervisors and team members.
- Participate in required training and professional development.
- Work collaboratively while maintaining the ability to manage an independent caseload.
Your Typical Day
A typical Care Coordinator may:
- Visit 3–4 members per day.
- Meet members in their homes or community settings.
- Help a member schedule a medical appointment.
- Coordinate with a healthcare provider after a hospital discharge.
- Connect a member with housing, food, transportation, or behavioral health resources.
- Follow up with a member who has been difficult to reach.
- Document services and update the member's care plan.
- Collaborate with the care team to address complex needs.

Minimum Qualifications
Education & Experience
Preferred:
· Bachelor's Degree of Social Work, Psychology, Social Services, Human Services, or Behavioral Science.
- Licensed Certified Chemical Dependency Professional
- Master's Degree in Social Work, Psychology, Social Services, Human Services, or Behavioral Sciences.
- Licensed Practical Nurse
- Licensed Mental Health Counselor
- Licensed Marriage and Family Therapist
- Licensed Clinical Psychologist
OR
Minimum:
· Indian Health Service (IHS) Certified Community Health Representative CHR)
- At least two years of experience working with high-risk, vulnerable, or underserved populations.
Relevant Experience
Experience working with one or more of the following populations or areas is highly valued:
- Medicaid members
