Job Description
JOB SUMMARY
The Oncology Nurse Navigator is a licensed registered professional nurse who is a clinical expert in oncology. The purpose of the position is to serve as the initial and primary point of contact for all new oncology patients and is responsible for ongoing assessment and comprehensive coordination of care across the continuum of care, working with a multidisciplinary team to identify and remove barriers to care and ensure optimal clinical outcomes for the patient from diagnosis to Survivorship.
ESSENTIAL JOB RESPONSIBILITIES Responsible for management of patient flow, triage, and direct patient care for every new cancer patient referred to LMH Medical Oncology in an effort to expedite timely care from diagnosis to Survivorship. Additionally, works with financial counselors to ensure compliance with No Surprise Act; i.e, assists with identifying treatment regimen and all charges contained therein prior to the start of treatment. Ensure new patients are scheduled with the appropriate multidisciplinary provider within the right timeframe, assists with coordinating needed diagnostic tests, and tracks these appointments/results to expedite start of treatment and throughout the continuum of care. Acts as a consultant to other members of the nursing and clinical staff for patients with complex health problems requiring specialized nursing care. Coordinates and prepares Survivorship Program; i.e. preparation of End of Treatment document, coordination of follow-up care and referrals based on post treatment needs and outcomes as indicated by NCCN guidelines. Responsible for disease and drug specific education for the patient and family Responsible for the weekly planning, coordination, and documentation of the general and disease specific Tumor Boards Provides eductional and emotional support to the patient throughout their course of treatment and assists with barriers to care, often referring to available supportive services.
Crosses care settings, develops relationships with referring physicians/offices to promote health, healing, and ensure follow-up and treatment plan is communicated Actively pariticipates in quality improvement initatives and keeps data and prepares reports to assist with ongoing Oncology accreditation through the American College of Surgeons Commission on Cancer and NAPBC. Participates in community outreach and education events Performs other duties as assigned.
JOB QUALIFICATIONS
Required :
- Current Registered Nurse license to practice in Kansas
- Current OCN, ONS Biotherapy/Chemotherapy Certification
- Current Basic Life Support for Healthcare Providers (BLS) Minimum of five (3) years of oncology RN experience
Preferred :
- Bachelor of Science in Nursing (BSN)
