The Clinical Operations Coordinator (RN) supports acute patient access activities within the Patient Stewardship & Navigation (PSN) Hub, including transfer coordination, provider consult triage, and tele-care facilitation. This role collaborates with referring providers, receiving teams, capacity management, and operational partners to ensure efficient coordination, accurate documentation, and timely communication across patient access workflows. The Coordinator applies clinical judgment to support triage decisions, facilitates coordination across teams, and contributes to safe and effective patient movement within a 24/7 operational environment.

Responsibilities
Essential Functions
- Coordinates incoming patient transfer, consult, and tele-care requests from referring providers and external facilities
- Gathers and documents patient clinical information to support triage, routing, and placement decisions
- Applies clinical judgment to assist in determining appropriate level of care and service line
- Facilitates communication between referring providers, receiving physicians, and operational teams to support timely decision-making
- Supports coordination of patient transfers, including prioritization based on clinical urgency and operational factors
- Coordinates and facilitates tele-care encounters, including initiation, provider connection, and documentation
- Documents all interactions and patient information accurately in designated systems (e.g., EPIC, PACS, Smart Web, Teams, and related platforms)
- Maintains ongoing communication with internal and external stakeholders to ensure continuity and follow-through of patient access workflows
- Monitors patient status and communicates updates or escalation needs to appropriate teams as required
- Collaborates with capacity management, hospital operations, and service line leadership to support patient flow and throughput
- Participates in data tracking and reporting activities related to transfer, consult, and tele-care workflows
- Provides support for process improvement initiatives and workflow optimization within the PSN Hub
- Assists with onboarding, training, and support of team members as assigned
- At the discretion of department operational and patient care needs, this position is required to work rotating schedules, which may include variable hours, weekends, nights, and holidays to meet the staffing and patient care demands of a 24/7 complex health system. Regular, reliable, and punctual attendance during assigned shifts is considered an essential function of the role.
Knowledge / Skills / Abilities
- Strong clinical assessment and triage skills in high-acuity or fast-paced environments
- Ability to prioritize multiple tasks and manage competing demands in real time
- Effective verbal and written communication skills with providers and interdisciplinary teams
- Ability to function effectively in high-pressure situations requiring rapid decision-making
- Knowledge of clinical documentation standards, privacy regulations (HIPAA), and patient confidentiality practices
- Proficiency in electronic systems and platforms used to support patient coordination (e.g., EPIC, PACS, Teams)
- Strong organizational skills and attention to detail
- Ability to collaborate across departments and work effectively within a team environment
Qualifications
Required
- Minimum 3–5 years of clinical experience in an acute care setting (e.g., ICU, ED, or equivalent)
- Bachelor of Science in Nursing (BSN)
Licenses Required
- Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
- Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.
Additional license requirements as determined by the hiring department.
Qualifications (Preferred)
Preferred
- Experience in transfer center, access center, acute patient placement, or hub operations within a high-acuity clinical environment
- Master of Science in Nursing (MSN) or enrollment in graduate nursing program
- Certification in care coordination, case management, or nurse navigation (e.g., ACM, CCM, ANCC board certification)
- Demonstrated experience working across multi-site or regional care networks in an acute or critical care context
- Experience with telemedicine platforms and virtual acute care coordination
