At Centennial Healthcare and Rehabilitation, we provide post-acute services, rehabilitation services, skilled nursing and short/long-term care. We are seeking LPNs FT 11-7 to join our dedicated team of healthcare professionals.

LPN Responsibilities include but are not limited to:
- Assesses, plans direct and evaluate total nursing care as determined by the residents/patients age-related physical, psychosocial, and cultural needs.
- Following established standards, policies, procedures, and resident’s/patient’s care plan.
- Handles emergencies in a prompt, precise, and professional manner. Performs CPR as required.
- Demonstrates an ability to remain calm, perform effectively and professionally during peak periods of activity and emergencies.
LPN Qualifications:
- LPN in the State of PA
- 1 year of nursing home experience
*Rotating weekends required*
