Job Description
We are seeking Clinical Nurse Specialists (CNS) to become Women’s Health Care Coordinators to identify and intervene in women’s health issues, by developing relationships with physicians and clinical staff, by creating educational programs for patients and professionals, and by advocating for patients through individual case management to ensure continuity of quality care along the healthcare spectrum.
These are newly created positions for experienced Clinical Nurse Specialists to assist in the ongoing development and implementation of the Women’s Health Care Coordination program, which promotes the CarePoint mission of improved access of individuals to quality, coordinated healthcare in their communities, in their hospitals, and in their homes.
Responsibilities include, but are not limited to:

Coordinating patient access to diagnostic and interventional services as well as clinical providers by assisting with scheduling and pre-certification of diagnostic and interventional services required based on screening results, standardized orders and physician referrals.

Responding to consultation based upon diagnostic abnormality by either a pre-visit phone assessment or on-site meeting with the patients, and providing counseling to patients with abnormal results or an identified need for additional diagnostic studies.

Developing a patient-specific plan of care based upon the diagnostic results and conducting clinical teaching based upon the individual's needs by talking with the patient, either in person or on the phone, to assist with any questions or concerns the patient may have throughout the diagnostic/treatment process.

Coordinating interactions with the multidisciplinary treatment teams, by maintaining a tracking mechanism for the patient through the diagnostic process to ensure centralized medical records documentation access to all the patient's healthcare providers, including physicians, pharmacists, dietitians, radiologists, physical therapists, counselors, and/or other women's health specialists.

Tailoring individual patient education portfolios as well as overseeing multimedia education for women's health programs at the CarePoint Family Health Centers in local New Jersey communities.
Qualifications
- New Jersey RN license
- MSN, Masters of Science in Nursing
- CNS, Clinical Nurse Specialist certification
- BLS / CPR
- 3+ years clinical experience in Women's Health
