Job Title: Clinical Site Manager, RN
Location: Sunflower Communities – Nagel Assisted Living
Compensation: $140,000 - $160,000 + 10,000 Sign-on Bonus!
Schedule: Monday - Friday (8:00am - 5:00pm) + Weekends and On-Call responsibilities.

Job Summary:
Nagel Assisted Living, a part of Sunflower Communities, is seeking a Clinical Site Manager (RN) to oversee the daily clinical operations of our assisted living community. This role ensures that residents receive high-quality care while maintaining compliance with Minnesota Assisted Living License regulations. The Clinical Site Manager will supervise and train staff, collaborate with interdisciplinary teams, and foster a therapeutic, safe, and welcoming environment for residents.
Key Responsibilities:
Clinical Oversight & Compliance:
- Ensure compliance with Minnesota Assisted Living License regulations and be available for state and federal licensure surveys.
- Conduct resident assessments upon move-in and ongoing to ensure their care needs are met.
- Develop, implement, and update care plans and service agreements in a timely manner.
- Oversee medication management, ensuring proper administration, documentation, and compliance.
- Investigate and report incidents of suspected resident neglect or abuse.
- Maintain accurate documentation related to resident care, assessments, and billing.
Supervision & Staff Development:
- Supervise and support licensed and unlicensed personnel, ensuring high standards of resident care.
- Train new employees and provide ongoing education on assisted living procedures and resident needs.
- Conduct competency evaluations and assist with staff performance assessments.
- Organize and lead at least 8 hours of in-service training annually for staff development.
- Monitor staff compliance with medication administration and resident treatment protocols.
Collaboration & Emergency Response:
- Serve as the primary clinical liaison between residents, families, case managers, and physicians.
- Communicate resident updates and care changes with interdisciplinary teams.
- Be available on-call for medical emergencies and changes in residents' conditions.
- Assist with therapeutic programs and activities as needed to maintain a positive environment.
Qualifications
- Active RN license in the State of Minnesota (unrestricted).
- Minimum of 3 years of experience in a related healthcare setting.
- Supervisory experience preferred but not required.
- Proficiency in electronic medical records (EMR) systems and general computer software.
- Valid driver’s license (if driving is required).
