Be the Best Hospice Aide Coordinator You Can Be
If you meet these qualifications, we would love to meet you:
- High school diploma required, with current certification as a Nurse’s Aide, Certified Home Health Aide (CHHA), or Certified Geriatric Nurse’s Aide (CGNA); CPR certification preferred (required in Georgia).
- Must have at least 2 years of experience in hospice care; CHAPNA (Certified Hospice and Palliative Nursing Assistant) certification preferred.
- Proficiency in Microsoft Office and EMR systems, with strong written and verbal communication skills and the ability to follow directions effectively.
- Must have reliable transportation, a valid driver’s license, and meet the physical demands of the job including frequent standing, walking, lifting (up to 200 lbs with assistance), bending, reaching, and handling medical supplies.
- Must be able to work in varied conditions including occasional exposure to heights, dampness, chemicals, dust, radiation, and noise.
- Demonstrates maturity, compassion, and the ability to handle the emotional and physical demands of hospice care; committed to the TRUE HOPE values and serving as a role model to all staff.
Responsibilities:
- Coordinate and assign Hospice Aide (HA) schedules and visits in collaboration with nursing leadership to ensure alignment with patient care plans and staffing needs.
- Serve as a mentor and liaison for HAs, supporting field training, orientation, competency reviews, and acting as a voice on committees such as QAPI.
- Oversee medical supply management, including ordering, distribution, cost control, and supply room organization to support efficient patient care.
- Provide direct patient care when needed, including personal hygiene, mobility support, elimination assistance, simple procedures, and documentation, based on the interdisciplinary plan of care.
- Collaborate closely with the interdisciplinary team, clinical staff, facilities, and families to ensure coordinated, compassionate care and support for HAs in the field.
- Participate in or take on additional responsibilities as assigned, which may include carrying a patient caseload or supporting within the scope of other clinical licensure.
