JOB SUMMARY:
- Plans, coordinates and facilitates the daily activities of the Social Workers and Social Work Associates on the QCIPN team to ensure all activities are performed in accordance with established policies and procedures.
- Assists with clinical program operations, care coordination efforts, and partners with QCIPN Executive, Operational and Quality teams to coordinate performance improvement initiatives based on the evolving strategies and goals of the QCIPN.
- Provides support and direction to department and serves as a resource to the staff.
- Ensures quality service and patient care in the department.

- Licensed Clinical Social Worker preferred.
- Current BLS certification.
- Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.
- Driver's abstract or evidence of good driving record may be required based on area of assignment.
Experience:
- Three (3) years Social Worker experience in ambulatory healthcare setting, preferably geriatrics, hospice/palliative care, behavioral health or chronic disease management.
- Prior supervisory/leadership experience.
