INTRODUCTION
Innovative Health Care Concepts (IHCC) is seeking an energetic, self-motivated individual for our Targeted Service Coordination program based from our Pocatello office. We are looking for someone who can manage multiple tasks, foster relationships with clients and the community, and has effective communication skills.
This is a professional level position working closely with people with intellectual and developmental disabilities. Our current opening can be part-time with an opportunity to grow into full-time in the future.
BASIC QUALIFICATIONS
Applicant must have at least a Bachelor’s degree in a human services field. Previous experience working with individuals with developmental disabilities is preferred but not required.
PAY
Hourly wage is paid per billable hour.
- Full-Time is 30 Billable hrs/wk
- Part-time is 10 - 29 Billable hrs/wk
- Training wage: $22/hr
- Billable hour wage: $23 - 24/hr depending on experience
Responsibilities
Job duties include:
- Monitoring and coordinating services as requested
- Developing service plans and assessments
- Participating in monthly team meetings with other providers
- Documenting services provided and completing required paperwork on time and accurately
- Traveling to client homes and provider agencies
- Attending monthly staff meetings and trainings
