Mid-Level Providers work in conjunction with a physician medical director and the Center Director to coordinate the care of patients receiving hormone replacement, targeted wellness (such as management of sleep apnea, weight control, hypertension, low libido). Candidates must have excellent medical and academic credentials, be technologically savvy, and able to effectively manage staff and operations at their location.
Qualifications
- Active Illinois NP or PA license in good standing without disciplinary actions
- Current DEA license-REQUIRED
- Must be able to train Monday-Friday for 10 consecutive days (2 weeks) following the clinical hours

We offer 2 full weeks of paid training.
Hours of operation:
- Monday: 8:00 AM - 5:00 PM
- Tuesday: 8:00 AM - 7:00 PM
- Wednesday: 8:00 AM - 1:00 PM
- Thursday: 7:00 AM - 5:00 PM
- Friday: 8:00 AM - 5:00 PM
- Saturday: 8:00 AM - 12 PM
- Sunday: Closed
This position will primarily be scheduled To PRN in Illinois. You will float to Hoffman Estates, Westmont and Burr Ridge.
Compensation:
- Saturdays are $400 per 4-hour shift, minimum of 2 per month, 3 Saturdays for 5 Saturday week months.
- Weekday shifts are $65 per hour.
