INTRODUCTION
Franciscan Health Crawfordsville Campus is seeking Licensed Practical Nurses (LPN) who focus on home healthcare. This role involves providing direct patient care, educating caregivers, and participating in the patient’s care team. LPNs must accurately assess needs, provide appropriate interventions, and revise care as needed under the supervision of a registered nurse. WHAT YOU CAN EXPECT
- Implement nursing plan of care at the direction of the registered nurse.
- Provide home-based patient care based on physician orders and the established nursing plan of care.
- Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient’s treatment needs, to achieve defined goals for improved patient outcomes.
- Conduct patient psychosocial assessments, to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.
- Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.
- Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.
QUALIFICATIONS
- Associate's Degree or Diploma from Accredited Licensed Practical Nurse Program - Required
- 1 Year Acute Care, Long-Term Care or Home Health Experience - Required
- 2 years Home Health Experience - Preferred
- Licensed Practical Nurse (LPN) - State Licensing Board - Required
- Cardiopulmonary Resuscitation (CPR) - American Heart Association - Required
- Driver's License - Required
- Auto Insurance - Required
