The Hospice Clinical Coordinator provides support to the clinical field staff by coordinating patient care, provides support in scheduling of patient visits and staff work schedules, and is a resource to the clinical manager, clinicians, and patients. The Hospice Clinical Coordinator works under the direction of the Clinical management team and reports to the Chief Clinical Officer.
This is a full time 7am-3:30pm and is located in the Wakefield RI location primarily but may need to cover Providence or Attleboro if needed for coverage.
What will you do as a Hospice Clinical Coordinator?
- Works closely with Patient Clinical Manager and Hospice Service Coordinators.
- Supports patients, families, and clinicians.
- Scheduling of staff visits per the patient care plan as needed.
- Performs chart audits.
- Utilizes reports and EMR features to monitor, track and improve clinical, quality and financial performance metrics.
- Monitors field staff caseloads and ensure field staff scheduling visits meet regulatory timepoint requirements.
- Follow up with patients/families and facilities as directed by the Patient Clinical Manager.
- Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through participation in continuing education and in-service classes. Attends all mandatory in-services.
- Consistently apply the organizations values in all aspects of work, complying with established Hope Health mission, policies, procedures and objectives in quality assurance, safety, environment, infection control and HIPAA compliance.
- Other duties as assigned.
