Matheny Medical and Educational Center has career opportunities available for a Full Time Licensed Practical Nurse Coordinator.
The LPN Coordinator is responsible for overseeing the healthcare needs of individuals living in Matheny’s Community Residences. This position coordinates medical care, administers medications, ensures compliance with state regulations, and collaborates with healthcare providers, families, and interdisciplinary teams to promote the health, safety, and well-being of residents.
Responsibilities of this position include:
- In conjunction with the Group Home Managers and RN Supervisor, provides clinical and managerial oversight of the Group Home LPNs and SRT LPNs.
- Coordinate and oversee healthcare services for residents in the group homes.
- Audit and maintain accurate and timely documentation, medication administration records (MARs), and incident documentation.
- Schedule and coordinate medical, dental, therapy, and specialty appointments.
- Ensure follow-up care and implement physician recommendations.
- Train and supervise DSPs on medication administration, infection control, emergency procedures, and health-related protocols.
- Monitor medication inventory, order refills, and ensure proper storage and disposal of medications. Follow up when necessary.
- Ensure compliance with state licensing regulations and agency policies.
- Collaborate with families, guardians, case managers, pharmacists, and healthcare providers to coordinate comprehensive care.
- Accompanies and transports residents to appointments and outpatient visits.
- Reviews meal plans to verify delivery of medically prescribed and proper nutrition.
- Provides “hands on care” to the residents, including activities of daily living (ADLs), personal care, and assisting residents with eating.
- Assist the group home leadership team with orienting new DSP and LPN staff for the group home routines.
- Conducts education with DSPs and LPNs on each resident’s medical conditions, treatments, and medically prescribed devices.
- Confirms physician orders are implemented properly and accurately.
- Develop staff schedules and ensure adequate nursing coverage.
