Description
PeaceHealth is seeking a LPN Chronic Disease Coordinator - Care Mgmt for a Full Time, 1.00 FTE, Day position.
The salary range for this job opening at PeaceHealth is $28.73 – $43.09.
The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.
Job Summary
Coordinates integrated care delivery across selected populations to improve clinical and financial outcomes through the implementation of health and disease management programs. Functions as a core member of the interdisciplinary care team and may fulfill several roles and responsibilities within scope of practice.
Details of the Position
- Coordinates and integrates care of selected populations by managing chronic disease registries and conducting outreach using various program delivery methods. Evaluates reports and acts on outlier data based on criteria set forth by the PHMG Quality Committee, appropriate regional disease management programs and/or performance improvement committee(s).
- Under care manager supervision, case manages a panel of patients, including implementing care plans and utilizing tracking tools, coaching for activation, providing education, patient/family support and coordinating with other care providers and resources.
- Works with discharge planners and coordinates transitions of care, arranging warm handoffs between the clinic and hospitals, emergency departments or care facilities. Facilitates the sharing of information across people, functions, and sites.
- Performs initial health screening per established protocols, resolves patient inquiries within scope, and escalates patient inquiries beyond scope to RN Care Manager via warm handoff.
- Participates as a proactive team member of a patient-centered medical home.
- Performs other duties as assigned.

- Fine motor skills to be able to grasp and control medical equipment and perform precise procedures.
- Push/Pull: Frequently up to 45 lbs. force (i.e., WOW, medical carts).
- Perform hands on CPR at least 20 mins using 100-125 lbs. of force (National Assoc. of EMS Physicians and AHA) Frequency should be seldom or occasionally.
- Ability to move around area with frequent sitting.
- Bending/stooping/squatting/reaching/kneeling frequently.

Lifting (non-patient) up to 20 lbs. occasionally and 5 lbs. frequently.

Exposure to biohazard, body fluids and airborne particles.
