Job Title: Project Coordinator – Tri-County Health Improvement Coalition
Department: Community Impact
Reports to: Vice President of Community Impact

Summary
The Project Coordinator leads a multi‑sector coalition—including public health agencies, health systems, and community‑based organizations—to implement the Community Health Needs Assessment (CHNA) and Community Health Improvement Plan (CHIP) using the Mobilizing for Action through Planning and Partnerships (MAPP) 2.0 framework. The role ensures a collaborative, equity‑centered process that engages partners, residents, and stakeholders in assessing community health needs, identifying priorities, and advancing strategies for improvement.
The Project Coordinator reports to United Way Fox Cities Vice President of Community Impact, and is responsible to the Coalition & Coalition Steering Committee.
Key Responsibilities
- Coalition Leadership & Facilitation — Convene, coordinate, and support a diverse coalition of public health, healthcare, and nonprofit partners; facilitate meetings, workgroups, and decision‑making processes aligned with MAPP 2.0.
- Project Management — Develop and manage project timelines, deliverables, and work plans for the CHNA/CHIP cycle; track progress and maintain documentation.
- MAPP 2.0 Implementation — Guide partners through each MAPP 2.0 phase, ensuring fidelity to the framework, including community engagement, data collection, strategic prioritization, and action planning.
- Community Engagement — Plan and implement inclusive engagement strategies to build coalition membership, gather input from residents, especially populations experiencing health inequities.
- Data Coordination — Support collection, synthesis, and presentation of quantitative and qualitative data for assessments, reports, and presentations.
- Communication & Reporting — Prepare meeting materials, summaries, presentations, and public‑facing documents; communicate progress to partners and stakeholders.
- Relationship Building — Strengthen cross‑sector partnerships and maintain effective communication channels among coalition members.
- Equity Integration — Ensure health equity principles guide all aspects of the CHNA/CHIP process, including engagement, analysis, and strategy development.
- Continuous Improvement — Identify opportunities to enhance collaboration, streamline processes, and improve the effectiveness of the coalition’s work.
Qualifications
- Education — Bachelor’s degree in public health, health administration, social sciences, community development, or related field; Master’s degree preferred.
- Experience — 2–4 years of experience in public health, community engagement, coalition management, project coordination, or related work.
- Knowledge — Familiarity with CHNA/CHIP processes, community health planning, or the MAPP 2.0 framework (or willingness to learn).

- Ability to manage complex projects with multiple partners.
- Experience working with diverse communities and applying equity‑centered approaches.

Work Environment:
Work environment is flexible, predominantly in-person with remote capabilities as approved by manager. Work is primarily performed in an office environment. This position requires regular movement inside the office to access filing cabinets, office machinery, etc. Attendance at meetings outside of the office is occasionally necessary. The position also includes remaining in a stationary position a portion of the time, either standing or sitting, and occasional lifting of up to 20 pounds. There may be exposure to repetitive work on the computer. Evening and weekend hours may be required.
