How to Get Visa Sponsorship in Orthopedic Physical Therapist
Target hospitals and medical centers first
Nonprofit hospitals and university medical centers are cap-exempt H-1B employers, meaning no lottery or October start date restrictions. These facilities also have established visa sponsorship processes for clinical staff.
Emphasize your specialized orthopedic training
USCIS approves physical therapy H-1B petitions more easily when the role requires specialized knowledge in orthopedics, sports medicine, or post-surgical rehabilitation rather than general physical therapy.
Get your credentials evaluated early
Foreign DPT degrees need credential evaluation to establish U.S. equivalency. Start this process before job searching since employers want confirmation your education meets licensing requirements.
Research state licensing requirements
Each state has different physical therapy licensing requirements and processing times. Some states accept foreign clinical hours while others require additional supervised practice before full licensure.
Consider outpatient orthopedic clinics
Private orthopedic and sports medicine clinics increasingly sponsor H-1B visas for specialized physical therapists, especially those with manual therapy certifications or experience with specific patient populations.
Highlight outcome measurement experience
Employers value physical therapists who can demonstrate measurable patient outcomes and quality metrics. This clinical documentation expertise strengthens your specialty occupation case for USCIS approval.

Get Access To All JobsFrequently Asked Questions
Do I need a DPT degree for H-1B sponsorship as an orthopedic physical therapist?
Yes, USCIS requires a Doctor of Physical Therapy (DPT) degree for H-1B approval in physical therapy roles. A master's degree in physical therapy may qualify if it's equivalent to current U.S. entry-level requirements, but you'll need a credential evaluation to confirm equivalency.
Can foreign physical therapists get H-1B sponsorship without U.S. clinical experience?
Yes, but you must meet state licensing requirements before starting work. Many states require supervised clinical hours or additional examinations for foreign graduates. Some employers sponsor H-1B visas contingent on obtaining state licensure within a specific timeframe.
Are physical therapy H-1B petitions frequently approved by USCIS?
Physical therapy generally has good H-1B approval rates when the position clearly requires a DPT degree and involves specialized orthopedic knowledge. Generic physical therapy roles or those that could be performed by physical therapist assistants face more scrutiny from USCIS.
Which employers are most likely to sponsor orthopedic physical therapists?
Hospitals, rehabilitation hospitals, university medical centers, and specialized orthopedic clinics are the most common sponsors. Nonprofit healthcare employers are cap-exempt, avoiding H-1B lottery requirements and allowing flexible start dates throughout the year.
Can I work as a physical therapist on other visa types besides H-1B?
Yes, if you qualify. Australians can use E-3 visas, Canadians and Mexicans can use TN visas for physical therapist positions. O-1 visas are possible for physical therapists with extraordinary ability in specialized areas like sports medicine or research.
What is the prevailing wage requirement for sponsored Orthopedic Physical Therapist jobs?
U.S. employers sponsoring a visa must pay at least the prevailing wage, which is what workers in the same role, area, and experience level typically earn. The Department of Labor sets this rate to make sure companies aren't hiring foreign workers simply because they'd accept lower pay than a U.S. worker. It varies by job title, location, and experience. You can look up current prevailing wage rates for any occupation and location using the OFLC Wage Search page.
