The Behavior Health Consultant is a manager in the Behavioral Health department that is assisting to connect patients to care. They take warm handoffs and referrals from primary care, and provide brief interventions and, when appropriate, connect the patients being referred to ongoing behavioral health services.
- Provides direct social work services, which include crisis intervention, individual, and group therapy, in collaboration with other members of an interdisciplinary team and completion of relevant documents, assessments, and treatment/shared care plans.
- Assesses patients for behavioral health service needs via screening tools and clinical contact and makes linkages to care.
- Provides single contact crisis intervention to patients seeing a primary care clinician.
- Provides short term treatment to patients as assigned.
- Reviews and outreaches to patients who receive a referral from Primary Care.
- Participates in departmental quality assurance and CQI activities.
- Supervises clinical and administrative work of assigned staff.
- Provides clinical supervision for assigned staff.
- Analyzes and evaluates the quantitative and qualitative performance of assigned supervisees and completes probationary and annual performance evaluations for staff within expected time frames.
- Oversees orientation and training of new staff assigned.
- Other duties as assigned.
Qualifications
- Master's in social work required
- New York State LCSW or LCSW-R required
- Basic computer and internet navigational skills required
- Computer literacy with Windows-based operating systems and MS Office applications (Word, Excel, Outlook, PowerPoint) required
- Familiarity with electronic health records (EHR technology) preferred
- Supervisory experience in health care setting, preferably in multi-site ambulatory care setting
- Bi-lingual Spanish preferred
