Case Manager – Inpatient Mental Health Treatment Center
Haven West Memphis is seeking a compassionate and experienced Case Manager. We are seeking a compassionate, organized, and motivated professional with behavioral health case management experience to help coordinate care and support patients throughout their treatment journey. Candidates must have prior experience working in a behavioral health treatment setting to be considered.
Schedule: Flexible hours Monday-Friday, either 8am-4:30pm or 8:30am-5pm, with one weekend rotation out of every six weeks.
Responsibilities
- Coordinate patient care from admission through discharge planning.
- Develop and maintain individualized treatment and discharge plans with the multidisciplinary team.
- Connect patients with community resources, housing, outpatient providers, and other support services.
- Facilitate referrals and coordinate aftercare appointments to ensure continuity of care.
- Collaborate with therapists, nursing, medical providers, and families to support treatment goals.
- Maintain timely, accurate, and compliant documentation in the electronic medical record.
- Advocate for patients while ensuring compliance with regulatory and accreditation standards.
Qualifications
- Prior Case Management experience in behavioral health is strongly preferred.
- Behavioral health treatment experience is required (inpatient, residential, psychiatric, or substance use treatment settings).
- Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred.
- Strong knowledge of discharge planning, community resources, and care coordination.
- Excellent communication, organizational, and problem-solving skills.
- Ability to work effectively within a multidisciplinary treatment team.
