I. Job Summary
The Medical Case Manager is expected to render direct services to a special target population requiring the coordination of multiple community services. The Medical Case Manager may be either medical or social work oriented with a working knowledge of multi-discipline team concepts in service delivery. Accuracy of assessment, appropriateness of case planning and interventions, and effectiveness of services rendered, will be reviewed with the program supervisor.
Specific responsibilities include:
1. To maintain a caseload of 45-50 medical case management clients.
2. Maintain direct contact through home visitation with each client at least one time during the time a client is being served as care coordination.
3. Conduct assessments, develop individual service plans (treatment care plans) to address medical adherence and compliance that utilize various community resources, act as advocate on behalf of clients, and monitor service delivery.
4. To reassess clients and update care plan through a direct visit (home or office) every 3 months to determine need for continued care coordination services.
5. to maintain accurate case records by documentation.
6.To contact agencies and make referrals in the best interest of client/family on ongoing basis.
7. to monitor, evaluate, and revise plans as indicated.
8. to arrange for all services identified in the case management plan.
9. to interact with other team members to facilitate productive and efficient team functioning.
10. To identify need for volunteers in clinic and home settings and to facilitate appropriate placement by communication need to appropriate personnel.
11. To receive direction and supervision from program supervisor regarding case management service delivery and organizational issues in a courteous, competent manner.
12. To prepare progress reports as required by Program Supervisor.
13. To share social service information and expertise regarding caseload with other team members and work in a collaborative mode with other team members.
14. To participate in other program activities including education activities.
15. To identify individuals who would benefit from a support group and arrange for their participation.
16. To lead or provide consultative role in support group for HIV-infected and/or substance abusing individuals or their family members.
17. To attend case conference/staff meeting to review care plan.
18. To develop and deliver presentations to community members regarding HIV infection and/or substance abuse on as –needed basis for the program.
19. To attend weekly staff meetings, supervision sessions, and group supervision/training sessions
20. Other miscellaneous assignments that help assure the success of this program.
Minimum Qualifications:
Bachelor’s Degree in a human service field such as mental health, psychology, nursing, social work or education
Good written and verbal communication skills
Good Time management and organizational skills
A working knowledge of the PLWHA population
An ability to work well with a diverse client and staff
