Job Summary:
Responsible for administering and directing appropriate nursing interventions as prescribed by a qualified physician or mid-level practitioner. Responsible for effective communication and accurate documentation. Displays professional behavior in accordance with Nursing Code of Ethics and CHS philosophy of service.
Hours of Position:
Full-time, nights (6:45p-6:45a)
KEY RESPONSIBILITIES
- Performing the initial nursing assessment, communicating findings with the assigned physician or mid-level
- Assessing the wound stage and measurements, documenting care plans, evaluating the patient status in regards to desired outcomes
- Identifying patient care situations that require intervention
- Participates in quality improvement initiatives, providing education to the patient and/or caregiver, and serving as patient advocate in the delivery and coordination of care
- Collaborates as needed with other healthcare providers, Wound Care Center Providers, the Program Director and Medical Director regarding patient needs
- Performs other duties as required
