Job Summary:
The Clinical BH/WM Staff Pharmacist is responsible for delivering comprehensive clinical pharmacy services that optimize medication therapy and support safe, effective, and fiscally responsible medication-use practices across all hospital units. This position reports directly to the Clinical Pharmacy Manager and the Director of Pharmacy and works collaboratively with interdisciplinary teams to enhance patient outcomes, advance medication management initiatives, and contribute to the development and continuous improvement of clinical programs, protocols, and quality-improvement efforts.
Essential Functions and Responsibilities:
- Participate in interdisciplinary care rounds to optimize medication therapy, ensuring safety, efficacy, and cost-effectiveness.
- Conduct clinical patient assessments, develop individualized therapeutic plans, and recommend pharmacotherapy adjustments based on clinical status.
- Provide direct clinical support to prescribers and nursing teams on medication management issues.
- Apply evidence-based clinical principles by critically reviewing medical literature and providing recommendations consistent with current standards of care.
- Respond to drug information inquiries from healthcare professionals, patients, and other stakeholders in a timely manner.
- Perform best-practice medication reconciliation at admission, transfer, and discharge to identify and resolve discrepancies.
- Provide patient counseling prior to discharge, ensuring patients and their caregivers understand the medication regimen.
- Ensure compliance with all local, state, and federal regulations governing medication use, storage, handling, and disposal.
- Lead or participate in performance and quality improvement initiatives aimed at advancing medication safety and reducing medication-related risks.
- Evaluate and manage non-formulary medication requests in accordance with institutional policies.
- Contribute to the Pharmacy & Therapeutics (P&T) Committee and other clinical committees by supporting formulary reviews, therapeutic interchange initiatives, and evaluations of new medications.
- Serve as a preceptor and mentor to pharmacy students and pharmacy staff.
- Develop and present educational programs, including workshops, in-services, and interdisciplinary training sessions.
- Participate in patient education initiatives and community outreach efforts.
- Promote strong communication and collaboration with physicians, nurses, and allied health professionals to facilitate coordinated patient-centered care.
- Demonstrate professionalism, ethical conduct, and leadership within interdisciplinary teams.
- Engage in continuing education, professional certification, and involvement in relevant professional organizations.
- Support the strategic advancement of clinical pharmacy services and advocate for best practices in medication therapy management.
- Perform Staff Pharmacist duties as required.

- Attend and contribute to clinical pharmacy and medical staff meetings, including the Patient Safety Committee, Pharmacy & Therapeutics (P&T) Committee, and Antimicrobial Stewardship Committee.
- Participate in medical rounds, interdisciplinary care (IDC) meetings, and other patient care discussions.
- Lead educational initiatives for healthcare and non-healthcare team members, including content related to pediatric pharmacotherapy when applicable.
- Conduct medication-use evaluations and collaborate on research projects to identify and implement opportunities for improved medication utilization.
- Serve on site-based and multidisciplinary committees as assigned.
- Organize, direct, and verify the work of supportive pharmacy personnel to ensure safe, accurate, and efficient delivery of medication therapy services.
- Ability to utilize technology platforms such as: Wolters Kluwer: Sentri 7, UptoDate, Lexi-drugs, etc.
- Act as a medication information resource for patients, physicians, nurses, and other members of the healthcare team.
Minimum Education/Certifications:
Required:
- Doctor of Pharmacy (Pharm.D.) from an accredited School of Pharmacy.
- Active New Jersey State Pharmacist License.
- Completion of a PGY1 Pharmacy Residency or a minimum of three years of relevant clinical practice experience.
Preferred:
- Completion of a PGY2 Pharmacy Residency in a relevant specialty.
- Board Certification through the Board of Pharmacy Specialties (BPS).
Position Type / Expected Hours
- (Variable) Shift options include: 7:00–3:30 PM, 7:30–4:00 PM, 8:00–4:30 PM, 10:30–7:00 PM, and 3:00–11:30 PM.
- Includes rotational weekend and holiday coverage.
- Includes combined responsibilities for both clinical and inpatient staff pharmacist coverage.
Physical Demands Analysis:
- Frequently required/prolonged sitting, standing, or walking.
- Manual and finger dexterity to operate computer & equipment.
- Walking, standing, and lifting of materials/equip up to 60lbs.
- Visual acuity required to read written and printed materials.
- Work performed in a climate-controlled, indoor setting.

CareWell Health provides a salary/hourly rate range for all open positions to comply with New Jersey Law. The rates listed for each position is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member’s base salary and/or rate, several factors may be considered as applicable (e.g., specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity). The salary range does not include bonuses/incentives, differential pay or other forms of compensation which can be discussed in detail with your talent acquisition specialist.
The salary/hourly rate range for this position is: $132,500 - $136,500 (exempt annually)
