- Evaluate the client’s current status and develop an individualized plan of care based on assessment findings
- Use nursing judgment to identify risks, changes in condition, unmet needs, and appropriate next steps
- Coordinate home care services and other service systems to support safety, well-being, and quality of life
- Conduct home visits and support clients in their own homes or other selected environments
- Serve as an advocate for clients and families as they navigate aging, changing health needs, and care decisions
- Review care management plans regularly and adjust services as client needs evolve
- Support client autonomy while guiding appropriate service utilization
- Maintain accurate documentation of client status, care coordination activity, and ongoing service needs
- Coordinate placement when needed, including identifying appropriate facilities, supporting the application and acceptance process, and helping with transition planning
- Ensure important planning conversations and documentation are addressed, including advance directives, power of attorney, wills, and burial plans, as appropriate

The strongest nurses in this role are able to:
- Build trust quickly with older adults and their families

What you bring
- Current Registered Nurse (RN) license in good standing

- Comfort with home visits and working in a community-based role
- Ability to communicate effectively with clients, families, providers, and community partners
- Strong organizational skills and timely documentation habits

Obtain Your State Nursing or Social Work License Early
BAYADA Care Manager roles typically require active state licensure before an offer is extended. Start your licensure application as soon as you have your credentials, since processing delays can push your start date past your OPT or grace period window.
Target BAYADA Offices in Underserved Markets
BAYADA operates across dozens of states, but rural and suburban markets often have the most persistent Care Manager vacancies. Offices struggling to fill caseloads have stronger business justification to pursue visa sponsorship than fully staffed urban locations.
Use Migrate Mate to Filter Active Care Manager Openings
Care Manager vacancies at BAYADA turn over frequently across service lines and states. Use Migrate Mate to filter openings by visa type so you're only reviewing roles where sponsorship has been confirmed, not guessing based on job description language.
Ask Recruiters About TN Eligibility If You Are Canadian or Mexican
Care Manager roles grounded in nursing or social work may qualify under TN visa categories, which have no annual cap and no lottery. Confirm with the BAYADA recruiter and a licensed attorney whether your specific role description supports a TN classification before applying.
Align Your Application to BAYADA's Clinical Documentation Standards
BAYADA emphasizes compliance and clinical documentation in its Care Manager hiring. Tailor your resume and interview responses to reflect experience with care plans, OASIS assessments, or similar home health documentation tools, which signals readiness for their regulatory environment.
Confirm H-1B Filing Timelines With HR Before Accepting an Offer
H-1B cap-subject petitions must be filed with USCIS in April for an October 1 start. If you receive an offer outside that window, ask BAYADA HR whether they'll support premium processing or whether a cap-exempt filing pathway exists through an affiliated entity.
Frequently Asked Questions
Does BAYADA Home Health Care sponsor H-1B visas for Care Managers?
Yes, BAYADA Home Health Care has sponsored H-1B visas for Care Manager roles. Care Manager positions typically qualify as specialty occupations requiring at least a bachelor's degree in nursing, social work, or a related clinical field. Sponsorship availability can vary by office location and current staffing needs, so confirming directly with the regional recruiter during the interview process is the most reliable approach.
Which visa types are commonly used for Care Manager roles at BAYADA Home Health Care?
H-1B and F-1 OPT are the most common pathways for Care Manager roles at BAYADA. F-1 CPT can apply if you're completing a clinical placement as part of your degree program. Canadian and Mexican nationals with qualifying nursing or social work credentials may also be eligible for TN status, which carries no annual cap or lottery requirement and can be a faster path than H-1B.
What qualifications or experience does BAYADA Home Health Care expect for Care Manager roles?
BAYADA generally expects active state licensure in nursing or social work for Care Manager positions, along with experience in home health, case management, or a clinical coordination setting. Familiarity with care plan development, OASIS documentation, and interdisciplinary team coordination strengthens your application. Some offices may require prior home health experience specifically rather than general clinical experience.
How do I apply for Care Manager jobs at BAYADA Home Health Care?
You can browse and apply for Care Manager openings at BAYADA through Migrate Mate, which filters roles by visa sponsorship type so you can identify confirmed opportunities without sorting through listings that don't apply to your status. When applying, confirm your visa situation early in the process since BAYADA's regional recruiters handle sponsorship decisions at the office level, and timelines vary by location and service line.
How do I plan my timeline if I'm on F-1 OPT and targeting a Care Manager role at BAYADA?
F-1 OPT gives you a limited window to secure employment and, if needed, transition to H-1B. If your OPT start and the H-1B cap filing window don't align, there may be a gap between your OPT expiration and an October 1 H-1B start date. Discuss this timeline with BAYADA HR early so they can plan accordingly and explore whether a cap-exempt route or bridging arrangement is feasible.
