Responsibilities
- Ability to screen patients and make referrals for services required
- Competency with EMR documentation
- Timely completion of patient request, follow up, form completion and pre-visit planning
Work Hours
Monday-Thursday 10:00AM-6:00PM
Friday 9:30AM-5:30PM
35 Hours Per Week
Qualifications
- NYS LPN Licensure (Grad of accredited LPN nursing program)
- Minimum 2 years of LPN experience in an Ambulatory health clinic
- Clinical Skills, phlebotomy, EKG, POCT, EMR documentation
- BCLS certification; venipuncture; EKG certification
- 1 year of LPN experience in an ambulatory health facility preferred
- Good computer skills required
- Good oral communication and interpersonal skills required
- Speaks, reads, writes English required to the extent of the position
- Bilingual Preferred
