Cleveland Clinic is seeking a Board Certified / Board Eligible Internal Medicine Hospitalist to join our rapidly growing post-acute care program within the Long Term Acute Care Hospital (LTACH) environment. This innovative role supports our mission to deliver world class care across the full continuum, ensuring safe, high-quality transitions from acute hospitalization back to the community.

- Post-acute and community partners to ensure continuity of care
BASIC QUALIFICATIONS
- MD or DO
