INTRODUCTION
The Clinical Health and Wellness Educator is responsible for supporting the clinical team and the enrollees by providing comprehensive education on health, wellness, emerging health concerns, disease management and resources for healthy living. The Clinical Health and Wellness Educator is part of the Medical Affairs Department and works collaboratively with the Utilization Management, Care Management and Medical and Psychiatric Medical Directors to design and implement monthly, quarterly and yearly educational offerings for our enrollee population. The Clinical Health and Wellness Educator is part of the team that supports our mission to provide access to care to our enrollees, improve health outcomes, provide preventative care and develop new programs in support of our approach to disease management and population health.
QUALIFICATIONS
Minimum Education
- MSN (Preferred)
- MPH (Preferred)
- BSN (Required)
Minimum Work Experience
- 5 years Experience in a clinical setting including direct care, clinical education, public health or other clinical role (Required)
- 2 years Community Health education, clinical education or public health education (Required)
Required Licenses and Certifications
- Registered Nurse District of Columbia 30 Days (Required)
JOB FUNCTIONS
Program Coordination
- Develops and implements enrollee educational program for HSCSN population and has the ability to work on projects with general direction and minimal guidance.
- Demonstrable ability to lead and deliver implementation of education program strategically through to completion with support from Clinical Operations team.
- Demonstrate competency in maintaining program documents using approved methodology, tools, and templates.
- Demonstrated ability to plan and organize programs including schedules, budgets, and program deliverables.
- Experience in working with a diverse population and ability to deliver education at a level that is understandable to the population we serve.
- Proven ability to handle multiple projects and meet deadlines.
Program support
- Support/participate in Medical Affairs/Clinical Operations.
- Ensure that education program is based on evidence of best practice.
- Accountable for participating in Medical Affairs/Clinical Operations performance improvement process.
- Participate in community activities, outreach activities, care management enrollee activities and provider outreach educational activities.
Program Evaluation
- Develops methods of evaluation to determine program effectiveness.
- Administers survey tool to obtain feedback from enrollees on educational offerings.
- Evaluates surveys and uses information to inform program content, makes changes as needed.
- Completes program description along with evaluation tool yearly.
- Conducts educational sessions for staff as needed or requested.
- Develops reports for the executive leadership team of activities, enrollee feedback, schedule of educational offerings etc.
- Works closely with the CMO to develop clinical priorities and educational offerings.
