Job Summary
Advanced Home Health Care - Licensed, Medicaid and Medicare certified Agency, located in West Hartford, has an immediate opening for Behavior / Med-Surg Licensed Practical Nurse (LPN). Full Time or Per Diem.
As an LPN with Advanced Home Health Care Agency you will provide assessments, treatment to patients and Med.Administration for behavior health patients in their homes.
Duties and Responsibilities:
Provides skilled nursing care to patients requiring professional nursing services in their homes.
Observes patients for changes in condition, reports changes appropriately to the physician, Registered Nurse, Clinical Supervisor, and other members of the home care team.
Initiates and maintains implementation of orders for medication and treatments as prescribed by the physician.
Assists and instructs patients, family members or other patient representatives in the implementation of the care plan necessary to promote the patient's health, safety, and independent living.
Participates in case conferences with other members of the health care team.
Minimum Requirements: Current state license as a Licensed Practical Nurse. Minimum of 2 year experience as a LPN in Home Health required. Demonstrated knowledge and skill in current nursing practice. Evidence of effective communication and organization skills Holds a valid driver's license.
