Civita Care Center at Cheshire is seeking a Physical Therapist Assistant (PTA) to join our rehabilitation department. This role offers the opportunity to help residents regain mobility, improve function, and achieve their rehabilitation goals in a supportive skilled nursing environment.
POSITION RESPONSIBILITIES:
- Deliver physical therapy treatments under the supervision of the Physical Therapist
- Assist residents with therapeutic exercises, gait training, balance activities, transfer training, and mobility interventions
- Support individualized treatment plans focused on improving safety and functional independence
- Monitor resident participation and communicate progress to the supervising PT
- Collaborate with nursing and interdisciplinary team members to support resident care
- Complete accurate and timely documentation
