The ideal candidate will have extensive experience in primary care, acute and chronic disease management, and a strong background in various medical specialties including geriatrics, pediatrics, emergency medicine, and behavioral health. This role offers the opportunity to provide comprehensive patient-centered care across diverse clinical settings, utilizing advanced EMR/EHR systems such as Epic and AdvancedMD. The Family Nurse Practitioner will be instrumental in delivering high-quality healthcare services, including preventive care, urgent care, post-acute management to our Franklin county rural health population.
Responsibilities
- Conduct thorough patient assessments including vital signs, physical examinations, and medical history reviews.
- Develop and implement individualized treatment plans across outpatient, inpatient, telehealth, and home care settings.
- Administer injections, IV infusions, catheterizations, and perform suturing with sterile technique.
- Manage acute pain, infectious diseases, chronic conditions such as diabetes or hypertension, and perform diagnostic evaluations using laboratory testing and medical imaging.
- Utilize EMR/EHR systems for accurate documentation of patient encounters and medical records.
- Provide health coaching on nutrition, medication adherence, lifestyle modifications, and disease prevention.
- Maintain compliance with HIPAA regulations and infection control standards while ensuring patient safety through airway management and sterile processing protocols.
Skills
- Proven expertise in diverse clinical areas including internal medicine, pediatrics (including neonatal intensive care), geriatrics (memory care & Alzheimer’s), emergency medicine (ER experience), behavioral health & psychopharmacology.
- Strong proficiency with EMR/EHR systems experience with CPT coding & ICD coding (ICD-9 & ICD-10).
- Ability to manage complex cases involving trauma (Level I & II trauma center experience), ICU/ PICU experience preferred.
- Familiarity with telehealth platforms for remote patient assessments.
- Knowledge of Workers’ Compensation law and post-acute care processes; experience with discharge planning and utilization management is advantageous.
- Excellent communication skills for working with diverse populations including children (toddler) and seniors (geriatrics).
