Position Summary

Life Health Care is seeking a compassionate and dependable Nurse Practitioner or Physician Assistant to join our growing healthcare team. This position may be performed virtually, in person, or through a hybrid arrangement. Both full-time and part-time candidates will be considered.

The ideal candidate is committed to providing respectful, patient-centered care to individuals from diverse backgrounds, including patients experiencing substance use disorders, mental health concerns, chronic health conditions, and barriers to traditional healthcare.

Essential Responsibilities

• Conduct comprehensive and focused patient assessments.
• Obtain and document medical histories, symptoms, medications, and treatment needs.
• Diagnose and manage acute and chronic health conditions within the provider’s scope of practice.
• Develop individualized treatment plans in collaboration with patients and the care team.
• Prescribe medications when appropriate and permitted by licensure and organizational policy.
• Provide medication management and follow-up services.
• Order and interpret laboratory tests, screenings, and other diagnostic services.
• Provide preventive care, health education, and risk-reduction counseling.
• Deliver services through secure telehealth platforms when working virtually.
• Complete accurate and timely documentation in the electronic medical record.
• Follow all HIPAA, confidentiality, clinical, prescribing, and documentation requirements.
• Coordinate care with physicians, nurses, medical assistants, therapists, recovery coaches, pharmacies, laboratories, and community partners.
• Make referrals to specialists and community resources when appropriate.
• Participate in case consultation, quality-improvement activities, and team meetings as needed.
• Respond professionally to abnormal test results, medication concerns, and changes in a patient’s condition.
• Maintain appropriate professional boundaries and provide trauma-informed, nonjudgmental care.
• Follow all applicable federal and Michigan laws, licensing standards, and organizational policies.

Preferred Areas of Experience

Experience in one or more of the following areas is preferred but not required:

• Primary care
• Family medicine
• Behavioral health
• Addiction medicine
• Medication-assisted treatment or medications for opioid use disorder
• Telehealth
• Community-based healthcare
• Care for underserved or justice-involved populations
• Chronic disease management
• Harm reduction and recovery-oriented care

Required Qualifications

• Current and unrestricted Michigan Nurse Practitioner or Physician Assistant license.
• Graduate of an accredited NP or PA program.
• Current national board certification.
• Current BLS or CPR certification.
• Ability to obtain and maintain professional liability coverage.
• Current DEA registration or eligibility to obtain one if prescribing controlled medications is part of the assigned role.
• Strong clinical judgment, communication, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Ability to use electronic medical records and telehealth technology.
• Commitment to patient dignity, confidentiality, cultural responsiveness, and recovery-oriented care.
• Ability to successfully complete required background checks, credentialing, and payer enrollment.

Virtual-Position Requirements

Candidates providing virtual services must have:

• A private and professional workspace that protects patient confidentiality.
• Reliable high-speed internet.
• The ability to safely and effectively evaluate patients through telehealth.
• Availability to communicate with on-site clinical and support staff.
• Compliance with all telehealth, prescribing, documentation, and patient-location requirements.
