SUMMARY: Program Associate oversees clerical support, the Individuals’ support and services through the NOW/COMP waiver programs for Individuals with Intellectual Developmental Disabilities.
SKILLS COMPETENCIES: Communication, reporting, time management, organizational, documentation and customer service.
DUTIES AND RESPONSIBILITES:
- Ensure that the Community Residential Alternative (CRA) homes’ needs are being met by the supervisor’s duties and responsibilities such as and not limited to grocery shopping, food menu, home maintenance and staffing.
- Monitor program services and changes as they occur.
- Report and document Individual and staff incidents according to policy and procedures and regulations.
- Home visits/ DDP Reports required for Individuals in their private residence in the Community Living Support (CLS) program.
- Ensure the timely filing and retrieval of documents in the CLS homes making sure that the Case Manager/Support Coordinator, staff and the Individuals are able to access the documents.
- Transport Individual(s) to community outings and/or scheduled appointment(s) in a company vehicle as needed.
- Assist with Individual Service Plan, ISP meeting and provide feedback as needed.
- Oversee Direct Support Professional’s documentation via paperwork, files (filing) as well as electronically.
- Maintain Individuals’ safety.
- Oversee emergency/disaster preparedness policy, plans and confirm emergency drills are conducted as scheduled in licensed and nonlicensed homes.
- Develop emergency/disaster preparedness plans with Direct Support Professional and family for the Individual in a nonlicensed home.
- Maintain compliance with all Individual charts: auditing, filing, and reducing to store in archives.
- Assist with scheduling for Individuals and Direct Support Professionals as needed and required.
OTHER DUTIES AS ASSIGNED:
- Filing, telephone communication, documenting in the call center, faxing, shredding, and correspond with emails daily.
- Conduct and record live fingerprint scan for Fieldprint..
- Provide direct support services as needed in field.
- Complete referral forms for services of potential individuals and fax to Agency/ Case Management Services.
- Other duties as required to maintain the overall operation of Family Bridge Healthcare Services.
QUALIFICATIONS:
- High school diploma or GED required, Associates degree in related health field preferred.
- Two years of DBHDD experience.
- Proxy trained.
- Driver’s license renewal is required prior to the expiration date.
- Behavioral support training, Mindset and/or Crisis Prevention, recertification is required prior to the expiration date.
- Clear DBHDD criminal background check.
- Current CPR/First aid certification, recertification is required prior to the expiration date.
- Intermediate reading and writing skills, verbal and written communication skills.
- Must be able to prioritize and multitask.
- Ability to meet deadlines.
- Must be able to address emergencies according to policies and procedures.
REQUIREMENTS
- Have an updated MVR on file renewal is required prior to the expiration date.
- TB and physical renewal are required prior to the expiration date.
- Minimum 16 hours of training annually.
- CPR/First aid certification renewal are required prior to the expiration date
