Summary
Supervises client care and caregiver performance through routine supervisory visits and ensures adherence to the plan of care and other pertinent regulations set forth by the Virginia Department of Health and other federal and state regulatory bodies.
Essential Duties and Responsibilities
- Performs supervisory visits within the required timeframe.
- Reviews the Plan of Care and makes changes as appropriate based on the needs of the client.
- Evaluates the effectiveness of home care services by obtaining feedback from the client and the caregiver.
- Coordinates client services and supports with the agency care specialists.
- Submits assessments and other required documentation outlining the services rendered as indicated in a timely manner.
- Participates in case conferences and interdisciplinary team meetings as scheduled.
- Works closely with the insurance care coordinators, facility case managers, and support coordinators regarding any planned or ongoing services.
- Discusses with the Administrator any problems concerning the clients or the services being provided to the clients and develops appropriate and effective solutions.
- Assists with or performs potential client intake, new client admission, resumption of care, discharge planning, and other assessment activities.
- Cooperates with other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive care plan.
- Participates in office meetings and staff development trainings.
- Assists with performance reviews of the caregivers and ensures competency of the same.
- Collaborates with the other departments such as human resources, compliance, and care team, based on specific projects assigned.
- Adheres to Federal, state and accreditation requirements.
- Maintains licensures and/or relevant certifications as required.
Minimum Job Requirements

Must be authorized to work in the United States.

Must pass criminal background checks and any Virginia-required screenings.

Must obtain an active, unencumbered RN license in Virginia or a compact state within 12 months from date of hire.

Active professional liability insurance with appropriate coverage.

Valid, unexpired driver’s license.
-
Negative PPD test or negative chest X-Ray if PPD was positive.
-
Active Basic Life Support (BLS) certification or higher.
Other Important Job Requirements
1. Education
At least an associate degree in Nursing. Bachelor’s degree in Nursing preferred.
2. Experience
Must have at least one year of experience working as a nurse in a clinical or public health setting.
Experience with compliance in healthcare regulations and relevant laws preferred.
3. Skills & Competencies
- Strong verbal and written communication skills.
- Effective interpersonal and leadership skills.
- Empathy and understanding of the needs of clients and caregivers.
- Problem-solving abilities and decision-making skills.
- Ability to maintain confidentiality and handle sensitive information professionally.
- Proficiency in Microsoft 365 and electronic health record platforms.
- Ability to work independently, manage multiple tasks, and meet deadlines.
4. Certifications:
CDP, CCM, ANCC certifications or similar, are a plus.
A criminal background check will be conducted once a conditional job offer is given.
Job Type: Full-time, Exempt/Salaried
Schedule:
- 40 hours/week
- Regular shift: 8am-4pm and PRN (as needed)
- Monday to Friday (may include weekends as needed)

Physical Requirements
While performing the duties of this job, the individual is regularly required to stand, walk, sit, use hands and fingers, handle and/or feel objects. Must use arms and talk and hear. The individual must be able to lift and/or move objects weighing up to twenty-five (25) pounds or more. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, and use of depth perception and ability to adjust focus.
Work Environment
Routine office conditions and occasional travel as necessary or as requested.
