About the Role
We are seeking a compassionate, skilled, and motivated Licensed Clinical Social Worker (LCSW) to provide psychotherapy and behavioral health services to residents with complex medical, neurological, and psychiatric needs.
This position combines direct clinical care, behavioral health assessment, therapeutic intervention, and interdisciplinary collaboration. Key Responsibilities
Provide individual psychotherapy to residents experiencing depression, anxiety, trauma, adjustment difficulties, and other mental health concerns.
Conduct comprehensive biopsychosocial and behavioral health assessments.
Develop individualized treatment plans with measurable therapeutic goals.
Provide evidence-based therapeutic interventions, including CBT, supportive psychotherapy, and other appropriate modalities.
Assess and address challenging behaviors, emotional dysregulation, and psychosocial barriers to care.
Collaborate with psychiatric providers, nursing, rehabilitation, and Social Services to coordinate behavioral health treatment.
Assist in developing individualized behavioral support plans and nonpharmacological interventions.
Provide crisis intervention, de-escalation, and emotional support as clinically appropriate.
Participate in interdisciplinary care conferences and provide clinical recommendations.
Support residents and families in adjusting to chronic illness, neurological impairments, disability, and changes in independence.
Maintain accurate, timely, and confidential clinical documentation in accordance with professional and regulatory standards.
Essential Skills

Required:

Experience in psychiatric care, behavioral health, neurorehabilitation, skilled nursing, or long-term care.
Experience or training in Applied Behavior Analysis (ABA), positive behavioral support, or behavioral intervention planning.
