We are seeking a compassionate and dependable PRN Hospice Social Worker to join our growing team serving Santa Fe and surrounding Northern New Mexico communities.
Responsibilities:
Provide initial Social Services assessments to hospice patients
Provide emotional support and counseling to patients and families
Assist with psychosocial assessments and care planning
Support patients and caregivers with community resources and end-of-life planning
Collaborate with the interdisciplinary hospice team
Assist with crisis intervention, grief support, and advocacy
Maintain timely and accurate documentation
Participate in IDG meetings as needed
Qualifications:
MSW required
Current New Mexico social work license required (LMSW)
Hospice, home health, palliative care, or healthcare experience preferred
Strong communication and interpersonal skills
Compassionate, team-oriented mindset
Valid driver’s license and reliable transportation
