We help patients recover at home after illness, injury, or surgery. We are hiring a Physical Therapist to provide skilled therapy in patients’ homes across central Oklahoma. This role is paid per visit, with an expected annual range of $85,000–$95,000 for a full caseload. You will receive a company car and gas card, so you are not putting miles on your personal vehicle.

- Re-evaluate regularly, revise the plan of care as needed, and document why a identified problem is not addressed when that is the case
- Start appropriate diagnostic, preventive, and rehabilitative procedures and refer to other disciplines when the patient’s needs require it
- Observe changes in condition and report them promptly to the physician and/or RN/Case Manager
- Teach patients and families home exercise programs, safety, use of prosthetic/orthotic devices, and durable medical equipment
- Coordinate care with nursing and other disciplines and attend care conferences as needed
- Complete accurate documentation within 24 hours of the visit
- Participate in quality improvement, in-services, and ongoing professional education
- Provide staff education when requested and arranged by the RN/Case Manager
- Flexible field schedule with travel to patient homes and community appointments
- Service area includes Oklahoma City, Edmond, Guthrie, Stillwater, Perry, and surrounding communities
- Work is performed in patient homes and in the community in a range of weather and home environments
- Graduate of a physical therapy program approved by CAPTE / APTA (or equivalent approved program)
- Current Oklahoma Physical Therapist license
- Current CPR
- Valid driver’s license
- Ability to work independently and as part of an interdisciplinary team
- Strong verbal, written, and interpersonal communication skills
- Ability to work a flexible schedule
- One year of clinical experience, ideally in home health
