Job Description:
A Brief Overview
Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, significant others, and other health team members while maintaining high standards of professional nursing. Provides high quality care to the following patient populations: Adult and pediatric patients experiencing a temporary change in condition needing home support. Coordinates all hospice services required for optimal end of life care.
What you will do
- Administer prescribed medications, management of medications-filling pillboxes, IV infusion pumps, and monitoring compliance.
- Answer patient, family, and provider calls and determine how to assist them.
- Collect lab samples per physician orders.
- Evaluate nursing intervention outcomes, conferring with other health care team members as necessary.
- Work as part of a health care team to assess patient needs, plan and modify care, supervision of aides and implement interventions.
- Enemas, IV or SQ button insertion.
- Macy Catheter insertion, Foley insertion, rectal tube insertion, wound vac, trach, ostomy care and teaching including management of Pleurx catheters.
- Conduct phone calls to family, providers, DME, nutrition services, pharmacy for care coordination, and other entities as needed.
- Assessment of patients, charting and reporting changes in patients conditions, such as uncontrolled symptoms and educating family, facilities and providers in regard to disease management and end of life and symptom management.
Qualifications
- Associate’s Degree (AA) Graduate of an accredited school of nursing Required
- 1-3 years Acute care experience Preferred
- Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
- Knowledge of the information and techniques needed to diagnose and treat human injuries, diseases, and deformities. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
- Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
- Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation;
- Knowledge of principles, methods, and procedures for diagnosis, treatment, and rehabilitation of physical and mental dysfunctions.
- Being aware of others' reactions and understanding why they react as they do.
- Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
- Adjusting actions in relation to others' actions.
- Talking to others to convey information effectively.
