Job Title: RN Case Manager (Registered Nurse)

Location: Hermiston, OR

Job Type: 13-Week Contract

Shift: Day Shift (8x5)

Pay Rate: $100 – $110 per hour

Job Summary:

We are seeking an experienced and detail-oriented RN Case Manager for a 13-week contract assignment in Hermiston, OR. The ideal candidate will be responsible for coordinating patient care, ensuring appropriate utilization of healthcare services, and collaborating with interdisciplinary teams to achieve optimal patient outcomes.

Key Responsibilities:

- Coordinate and manage patient care plans from admission through discharge
- Perform utilization review to ensure appropriate level of care
- Collaborate with physicians, nurses, and other healthcare professionals
- Facilitate discharge planning and transitions of care
- Communicate with patients and families regarding care plans and progress
- Ensure compliance with hospital policies, state, and federal regulations
- Document all case management activities accurately and timely
- Identify and address barriers to care and patient progress

Requirements

- Active Registered Nurse (RN) license (Oregon or compact preferred)
- Minimum 2+ years of Case Management experience
- Experience with Utilization Review (UR) and Discharge Planning
- Current BLS certification (required)
- Strong knowledge of healthcare regulations and payer guidelines
- Excellent communication, organizational, and problem-solving skills

Preferred Qualifications:

- Previous experience in acute care or hospital setting
- Case Management certification (CCM) preferred
- Familiarity with EMR systems
